Complete an Unstaffed Fitness Center Membership Application.
STEP 3
eFax the completed application and signed waiver to Midland Occupational Health at 866-275-6441.

The enrollment process may take up to 2 weeks. Please expect to receive an email directly from Security, notifying you of your access status.
